Job summary

Valor Healthcare, Inc. is seeking a Licensed Practical Nurse to join the Roane County/Harriman Community Based Outpatient Clinic team. The role involves delivering prescribed care, collaborating with the PACT team, and supporting patient education and care plans within a VA setting.

The position offers a weekday schedule, competitive compensation, and comprehensive benefits. Candidates should have an active LPN/LVN license, at least one year of ambulatory care experience, and VA-credentialing

Qualifications

Diploma or certificate of an accredited practical nursing program required.
Minimum one-year experience as an LPN/LVN in primary/ambulatory care; government/VA experience preferred.
Must be credentialed and in good standing through VA; valid license in applicable state.
Current BLS certification and ACLS as specified by VA contract.
Proficiency in written and spoken English; strong computer/EMR skills.

Responsibilities

Assist with patient care, examinations, and routine nursing assessments.
Administer medications as ordered and document in EMR.
Operate EKG and other diagnostic equipment; collect specimens as needed.
Maintain patient records in VISTA/CPRS and ensure privacy of veteran data.
Provide patient and family health education and coordinate with VA/community resources.
Support front desk tasks as needed and participate in care coordination with non-VA providers.
