Your Health Organization in the Myrtle Beach area is seeking a Registered Nurse to join our Visiting Nurse Nurse Case Manager team. The role visits patients at homes and facilities to coordinate care, initiate telemedicine visits, and support providers within the care plan.
You must have reliable transportation, be willing to travel daily, and work full time from 8am to 5pm, Monday through Friday. We support primary care, specialty services, and telemedicine within a comprehensive care framework.
We are seeking Registered Nurse to service our patients throughout the Myrtle Beach area The role of the Visiting Nurse Nurse Case Manager position is a critical part of the patients care team The nurse will visit patients and facilitate with appropriate provider The Nurse Case Manager will be required to travel to patients homes long term care facilities etc to evaluate patients and initiate telemedicine visits Provide quality healthcare in adherence to all applicable laws regulations and policies within the scope of practice Nurse Case Managers perform visits in homes and facilities ALF and ILFs in their designated service area You must have reliable transportation as travel is required daily This is a full time salary based working 8 hr shifts Monday Friday 8am 5pm.

Area of Responsibility Facilitate receiving all medical records from the patients primary provider and specialistsReview medical recordsComplete consents with patientsEnroll patients in Care Management if they meet eligibility criteriaInitiate a Care Management Plan of Care if the patient is eligibleCapture all diagnoses at the highest specificity by creating gaps and ensure they are acceptedComplete AWVs to be reviewed by the providerComplete cognitive impairment screeningsComplete Social Determinants of Health SDoH assessments andor screeningsComplete ACPs to be reviewed with the patient by the providerEvaluate for home health hospice palliative or consults with Your Health Specialty Division etcEvaluate for RPM devices resources or tools that may improve the patients quality of lifeCommunicate and coordinate careReconcile prescribed and OTC medications vitamins supplements herbal remedies and other treatmentsProvide post discharge educationEvaluate for adaptive equipment and DME Evaluate for safe environmentEvaluation of acute conditions or follow up from previous visitAppropriately and accurately document and log Care Management activitiesWork in conjunction with care team to keep the patients Care Management care plans up to dateOn call maybe required
Must be a licensed nurse Registered nurse preferred License must be in good standing with appropriate boardissuer A minimum of three 3 years clinical experience preferred Experience in community settings preferred Proven ability to effectively communicate and collaborate with interdisciplinary care teams patients and caregivers Strong written and verbal skills Basic computer knowledge Ability to manage and demonstrate effective leadership skills Should demonstrate good interpersonal and communication skills under all conditions and circumstances Ability to foster a cooperative work environment Team player with ability to manage multiple responsibilities and demonstrate sound judgment Must be able to work flexible hours and travel between offices facilities etc Must be a licensed driver with an automobile that is insured in accordance with state andor organizational requirements and is in good working order
