Your Health is seeking a Registered Nurse to join our Visiting Nurse team serving the Augusta area. The Nurse Case Manager will travel to patients’ homes, long-term facilities, and care settings to evaluate and initiate telemedicine visits, coordinating with providers for high-quality care.
This full-time role requires a valid RN license, 3+ years of clinical experience, and excellent communication. Travel to patients and flexible hours are expected; vehicle and insurance readiness are assumed.
The role of the Visiting Nurse (Nurse Case Manager) position is a critical part of the patient’s care team. The nurse will visit patients and facilitate with appropriate provider. The Nurse Case Manager will be required to travel to patients’ homes, long-term care facilities, etc. to evaluate patients and initiate telemedicine visits. Provide quality healthcare in adherence to all applicable laws, regulations, and policies within the scope of practice.
You must have reliable transportation as travel is required daily. This is a full time, salary-based working 8-hr shifts Monday-Friday (8am-5pm)
