Job summary

Legacy Hospice in Fredericktown, MO is seeking a compassionate Visit Registered Nurse (VRN) to provide in-home hospice care and support to patients. You will supplement RN Case Managers, perform focused assessments, and help manage care plans with families.

Responsibilities include administering meds, educating families, documenting thoroughly, and collaborating with the interdisciplinary team. A valid RN license in Missouri and BLS are required; hospice experience is preferred.

Qualifications

• RN license in the state of employment
• BSN and/or CHPN certification preferred
• Hospice nursing experience: 1 year minimum (2+ preferred)
• Active BLS certification
• Strong written and verbal communication
• Able to work with minimal supervision
• Valid driver’s license and reliable vehicle
• Pre-employment drug screening including marijuana testing

• Perform focused nursing assessments and address acute hospice needs as directed
• Ensure continuity of care through communication with the interdisciplinary team
• Complete initial and comprehensive patient/family hospice assessments
• Develop and revise individualized care plans with patient and caregiver
• Administer prescribed meds and educate families on care techniques
• Counsel patients and families on symptom management and end-of-life issues
• Maintain accurate, timely clinical documentation
• Assist patients with activities of daily living and comfort-focused care
• Respond promptly to changes in patient condition or caregiver concerns
• Collaborate with case managers to arrange supplies and services
• Participate in interdisciplinary team meetings and quality improvement initiatives

About Us

Legacy Hospice is seeking a compassionate and experienced Visit Registered Nurse (VRN) to join our dedicated team. In this role, you’ll provide vital care and support to patients in their homes, supplementing our RN Case Managers and ensuring timely responses to urgent needs.

What You’ll Do

• Perform focused nursing assessments and address acute needs of hospice patients as directed
• Ensure continuity of care through effective communication with the interdisciplinary team
• Complete initial and comprehensive assessments of patients and families to determine hospice needs
• Develop and revise individualized care plans in collaboration with the patient and caregiver
• Administer prescribed medications and treatments, and educate families on care techniques
• Counsel patients and families on symptom management and end-of-life issues
• Maintain accurate and timely clinical documentation
• Assist patients with activities of daily living and promote comfort-focused care
• Respond promptly to changes in patient condition or caregiver concerns

• Certified Hospice and Palliative Nurse (CHPN) and/or BSN preferred

• Active Basic Life Support (BLS) certification
• Strong written and verbal communication skills
• Self-motivated and capable of working with minimal supervision
• Valid driver's license, reliable vehicle, and proof of insurance
