All Seniors Foundation is seeking a Telehealth Nurse to deliver virtual clinical support and coordinate care for seniors in the Los Angeles area, with remote work possibilities. Qualifications

• Registered Nurse license in California.
• Experience in telehealth or home health nursing.
• Excellent communication, technology literacy, and assessment skills.

Responsabilités

• Perform remote patient assessments, triage, and symptom management.
• Provide education on chronic disease management, wellness, and preventive care.
• Coordinate referrals, follow-ups, and care plans with on-site providers.
• Maintain accurate telehealth documentation and comply with privacy regulations.
