Job summary

Devoted Medical Group, PC is seeking an experienced nurse practitioner or physician assistant to lead virtual diabetes care for seniors. You will work with a diverse care team to deliver focused assessments, create individualized treatment plans, and ensure high-quality diabetes management in a remote setting.

Applicants should have 5+ years in outpatient endocrinology/diabetes, multistate licensure, and strong telehealth/EHR proficiency.

Qualifications

5+ years in outpatient clinical practice, ideally endocrinology or diabetes focused clinic.
Minimum of 2 years of diabetes management and education.
Proficiency in telehealth technology and EHR systems.
Virtual care experience preferred; willingness to continue practicing clinical nursing and virtual visits.
Understanding of managed care and ability to assess STARS/HEDIS measures and coding for comorbidities.

Responsibilities

Conduct virtual diabetes assessments, interpret labs/imaging, and develop treatment plans with the care team.
Formulate diagnoses and plan diabetes management, including meds and lifestyle guidance.
Collaborate with PCPs, endocrinologists, pharmacists, social workers, and others to coordinate holistic care.
Provide clinical escalation support and participate in panel reviews for medical management guidance.
Ensure accurate records and compliance with privacy and telehealth regulations.
