9amHealth is seeking a licensed Registered Dietitian Nutritionist for a full-time remote Clinical Care Specialist role in the United States. The clinician provides diabetes, hypertension, hyperlipidemia, and obesity care through chat, phone, and video, and develops nutrition and lifestyle plans.

The role requires at least five years of direct patient care and may prefer clinical or telehealth backgrounds focused on chronic disease management; CDCES or BC-ADM certification is a plus.

Qualifications

• Licensed RDN with 5+ years of direct patient care.
• Prefer clinical or telehealth experience in chronic disease care.
• CDCES or BC-ADM certification adds focused credential for diabetes care.

Responsibilities

• Manage a panel of members seeking cardiometabolic care.
• Develop nutrition and lifestyle plans.
• Interpret health metrics and lab results; track progress.
• Create physician consults and coordinate with care teams.
• Document care activities in EMR; support via chat, phone, video.
