Position Summary

The APRN is a clinical role that will provide exceptional medical care to patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under the supervision of a physician. This safety‑sensitive position involves direct patient care and medication administration. The incumbent will report to the Regional Manager (or similar role) and will work closely with other healthcare professionals to provide quality care, collaborating with other clinical teams to support team function.

Responsibilities
  • Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of medical practice; knowing limitations and seeking consult when advisable; keeping abreast of current technologies and industry standards.
  • Under the supervision of a physician, provide care for patients with acute and chronic illnesses encountered in older adults.
  • Accountable for patient care and outcomes, seeking consultation from specialists when needed while remaining involved in detailed care of the patient.
  • Engage with hospitalists whenever a patient is in the hospital.
  • Meet all clinical obligations of the practice, including accreditation agency compliance, CLIA, drug prescription compliance, HIPAA, state and federal regulations, and safety regulations.
  • Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence‑based medicine and practice guidelines.
  • Follow all internal policies and procedures.
  • Be accountable for completing all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation.
  • Maintain complete and detailed records in the EMR of patients’ health care plans and prognoses, completed within 48 hours of each visit.
  • Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability.
  • Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Interpret data and diagnostic results for appropriate clinical action, including but not limited to blood gases, radiology, EKG, in collaboration with the supervising physician.
  • Exhibit behavior emphasizing compassion, quality, loyalty, and conscientiousness.
  • Establish and maintain effective working relationships with patients, their families, and team members.
  • Document continuing education per licensure and certification requirements.
  • Ensure patients are seen by the physician on every other visit.
  • Maintain patient confidentiality according to policy, procedure, and HIPAA requirements.
  • Enact proper chain of command for patient complaints.
  • Perform additional duties as assigned.
Position Requirements & Skills
  • Graduate of an accredited nursing school.
  • Certified as a nurse practitioner with current state licensure.
  • At least 1 year of prior professional nursing experience, including 1‑3 years as a primary‑care nurse practitioner.
  • Prior experience working with an electronic medical records system.
  • Provide 5‑star service in line with Better Health Group’s mission.
  • Skilled in traditional nursing disciplines of patient assessment and management.
  • ACLS/BLS certification required as mandated by state.
  • Demonstrate knowledge of appropriate patient care rationale.
  • Handle emergency situations calmly and effectively.
  • Read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
  • Effectively present information and respond to questions from diverse groups.
  • Define problems, collect data, establish facts, and draw valid conclusions.
  • Excellent written and verbal communication, teaching ability, and good telephone etiquette.
  • Working knowledge of Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real‑time collaboration.
Physical Requirements
  • Full range of body motion; handle and transfer patients, manual and finger dexterity, eye‑hand coordination.
  • Require standing, walking, pushing, bending, kneeling, and reaching arms‑length and overhead for prolonged periods.
  • Must lift and move patients and medical equipment as needed; lift/carry up to 15 lbs.
  • Corrected vision and hearing within normal range.
  • Ability to sit for extended periods.
  • Ability to operate a motor vehicle and maintain own transportation.
