Job summary

HMR Veterans Services in Walterboro, SC is seeking a Licensed Social Worker to plan, implement, and evaluate social services programs for residents, in alignment with the resident’s care plan and as directed by the Administrator.

You will develop and coordinate department policies, assist in discharge planning, and work with families to meet social, emotional, and welfare needs within a hospital-like setting for Veterans.

Qualifications

• Bachelor's degree from an approved school of Social Work.
• One year supervised social work experience in a health care setting.
• Ability to provide care appropriate to the age-related needs of residents.

Responsibilities

• Assist in planning, developing, organizing, implementing, and evaluating the social services programs of this facility.
• Develop, administer, and coordinate department policies and procedures.
• Identify and address medically related social and emotional needs of residents.
