Job summary

Qualifications

Responsibilities

  • Complete biopsychosocial assessments in collaboration with the Veteran and care team.
  • Provide case management and discharge planning with community resources.
  • Develop and implement treatment plans with the interdisciplinary team.
  • Offer short-term individual and group psychotherapy as clinically indicated.
  • Coordinate referrals to VA/community services and document interactions.
