Job summary

Qualifications

• Master's degree in social work from a CSWE-accredited program.

• PhD in social work not a substitute for MSW; licensure can be required for higher grades.

Responsibilities

• Conduct psychosocial assessments to determine Veterans and family needs.
• Provide individual, family, and group psychotherapy as part of treatment planning.
• Participate in discharge planning to ensure timely and appropriate transitions of care.
• Coordinate community-based services and referrals to VA programs and community agencies.
• Maintain documentation in CPRS and ensure proper handoffs during transfers of care.
• Collaborate with care team and participate in committee activities as needed.
