Upscale Home Care is seeking a Patient Care Assistant to provide high-quality in-home care for veterans and seniors in Cave Creek, AZ. You will work with a multidisciplinary team to deliver personalized care in clients' homes.

The role includes assisting with daily living activities, medication reminders, meal preparation, mobility support, and maintaining a safe home environment. Part-time and full-time schedules are available with competitive compensation.

Qualifications

• Education: High School Diploma or equivalent.
• Experience: 1 year in a similar role.
• Work schedule: Part-time and full-time availability.

Responsibilities

• Patient Care: Assist with daily living activities, provide medication reminders, and deliver personalized care to our clients.
• Communication: Communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination.
• Housekeeping: Maintain a safe and clean environment for our clients.
• Personal Care: Assist with bathing, dressing, and grooming.
• Mobility and Transfers: Assist with mobility and transfers as needed.
