Responsibilities

• Deliver comprehensive primary care services with an emphasis on health maintenance, disease prevention, patient education, and chronic disease management.
• Promote shared decision-making with veterans and their families to achieve optimal health outcomes.
• Implement and practice according to the PACT model, focusing on collaboration and coordination within the multidisciplinary healthcare team.
• Utilize evidence-based practices to enhance patient care and outcomes.
• Participate in continuous quality improvement projects to enhance care delivery.
• Utilize quality measurement tools to monitor and improve clinical performance and patient outcomes.
• Serve as a mentor and role model within the primary care team; engage in educational activities with residents and trainees.
• Coordinate care involving specialty care, mental health, and other services to ensure comprehensive healthcare delivery.
• Collaborate with providers to develop individualized treatment plans for veterans.
