Job summary

Penn State Health Milton Hershey Medical Center is seeking a Vascular Surgery Advanced Practice Provider (APP) to support inpatient and outpatient vascular care. The role involves a rotating monthly schedule between inpatient coverage and outpatient clinic duties, with no operative responsibilities.

The APP will collaborate with vascular surgeons, APPs, and nursing staff, performing assessments, coordinating care, and contributing to discharge planning and follow-up care for diverse vascular

Qualifications

• Physician Assistant graduate, eligible for PA-C certification/licensure.
• Current state licensure and national certification applicable to APP role.
• ACLS and BLS certification.
• Strong clinical assessment, communication, and care coordination skills.

Responsibilities

• Provide inpatient vascular surgery coverage, including evaluation and management of new and established patients.
• Respond to and manage vascular surgery consultations with the team.
• Perform daily patient assessments, develop and implement treatment plans, and coordinate ongoing care.
• Facilitate hospital discharges, including medication reconciliation, patient/family education, and follow-up planning.
• Provide outpatient vascular surgery clinic coverage, including patient evaluation, follow-up, and management of vascular conditions.
• Collaborate closely with vascular surgeons, APPs, nursing staff, and other team members.
• Communicate clinical findings, treatment plans, and changes in patient status to patients, families, and the healthcare team.
• Participate in care coordination and transitions of care across inpatient and outpatient settings.
