Responsibilities

• Care Gap & Population Health Management: Review and prioritize value-based care reports and identify patients needing preventive care.
• Transitional Care Management (TCM): Track hospital discharges and post-discharge outreach within timelines.
• Care Coordination & Patient Support: Schedule appointments and coordinate care between providers and services.
• Documentation & Chart Review: Review charts for missing data and maintain HIPAA compliance in the EHR.
• Health Plan & Quality Reporting: Track performance metrics and assist with audits and quality initiatives.
