Qualifications

Responsibilities

• Document discharge planning and progress toward goals.
• Consult with team members for discharge plans and follow-up.
• Maintain communication with multidisciplinary team about patient status.
• Assist with community resources referrals and discharge planning for patients and families.
• Support crisis de-escalation and intervention when needed.
• Assist with timely discharge facilitation and referrals to ongoing care.
