Job summary

Streamline Urology is seeking a Nurse Practitioner specializing in Urology to deliver expert care directly to patients in various post-acute settings. You will own a flexible caseload and work as an embedded clinical partner within nursing facilities, providing comprehensive assessments, treatments, and patient education.

The ideal candidate holds an NP license with family or adult-gerontology certification, and has experience in urology or related fields, with proficiency in EMR systems

Qualifications

• Valid NP license in good standing.
• Experience in urology or related specialties preferred.
• Proficiency with EMR/EHR systems including Point Click Care.
• Knowledge of CPT and ICD-10/ICD-9 coding standards for billing.

Responsibilities

• Round on-site at skilled nursing and post-acute facilities on a defined weekly schedule within your territory.
• Assess, diagnose, and manage urological conditions.
• Embed as a clinical partner to facility nursing teams.
• Conduct thorough patient assessments and vital signs monitoring.
• Perform diagnostic procedures such as ultrasound and catheterizations.
• Document all patient interactions in EMR per HIPAA.
