Glendora Hospital is expanding its care coordination team. The UR Case Manager will facilitate care along a continuum, coordinating discharge planning, and ensuring access to appropriate resources for patients in California.
The role requires an LVN license and a compassionate, team-oriented approach, with HIPAA compliance knowledge. The perfect candidate will collaborate with physicians, therapists, and community providers to optimize patient outcomes.

The UR Case Manager is responsible to facilitate care along a continuum through effective resource coordination through discharge to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. Shall have knowledge and experience of confidentiality and compliance with HIPAA regulations.
LVN license required.
Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for Glendora Hospital roles, including but not limited to shift differential and other special pay practices, etc. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that Glendora Hospital in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
