Responsibilities

  • Organizes workflow and appointments by coordinating with Care Managers and leadership to ensure health risk assessment data is collected and follow-ups scheduled.
  • Answer patient calls, field questions, and assist patients in making care manager appointments.
  • Schedules appointments and follow-up appointments via telephone for care management patients.
  • Reschedule or cancel appointments for care management patients and document patient information per confidentiality standards and HIPAA.

Job description

POSITION TITLE: Unlicensed Community Health Worker DEPARTMENT: Quality Management NATURE OF POSITION: This position is responsible for greeting patients and working with them in person, on the phone or through electronic correspondence to meet their needs regarding scheduling, data collection and updating patient information. This position will ensure accurate, timely and proper scheduling of nursing visits for various nursing/patient assessments. This position should have knowledge and understanding of the proper procedures for patients when verifying insurance, completing demographics, gathering health risk assessment data, and scheduling patients for initial and follow-up visits. Must be pleasant and effective in telephone conversation with patients and other employees. This position is an integral part of the healthcare team and must be knowledgeable of clinic procedures. In addition, he/she will maintain patient confidentiality and abide by The Clinic’s (COFMC) policies and procedures.

MINIMUM QUALIFICATIONS
  • High School graduate or equivalent required

RESPONSIBILITIES
  • Organizes workflow and appointments by coordinating with Care Managers and leadership to ensure health risk assessment data is collected, assigned follow-ups are scheduled, and receives instruction for communication to be called to patients.
  • Answer patient calls, field questions, and assist patients in making care manager appointments.
  • Schedules appointments and follow up appointments via telephone for care management patients.
  • Reschedule or cancel appointments for care management patients. Note all patient information according to confidentiality standards and HIPAA requirements.
  • Ensure adequate care manager visits are completed by following weekly goals.
  • Answers the phone and directs calls as appropriate.
  • Maintains care manager schedules by making proper appointments.
  • Schedules care manager visits, collects patient data and assures that all the appropriate clinical and insurance information is collected for the patient chart.
  • Participates in agencysponsored staff training events.
  • Always protects the confidentiality of patients and their records by complying with The Clinic’s patient confidentiality and HIPAA policies and procedures.
  • Provides guidance as needed to departmental employees regarding clinic policies and procedures.
  • Keeps workspace neat and organized.
  • Maintains a professional demeanor.
  • Exhibits a positive attitude and a commitment to the basic mission of the organization.
  • Exhibits caring qualities consistent with the values of a community health center.
  • Exhibits awareness of cultural factors and shows competence in reducing barriers to the provisions of health care services.
  • Performs other duties as assigned.
