Job summary

PacificSource seeks a qualified nurse or behavioral health professional to coordinate member care with physicians, nurses, social workers, and other professionals. You will assess health plan benefits and coordinate access to medical, community, or financial resources to promote outcomes.

You will perform utilization management, review services and documentation, and work with multi-disciplinary teams to ensure appropriate levels of care.

Qualifications

• Active, unrestricted nursing or behavioral health license required (RN, LPC, LMFT, LCSW or PMHNP).
• Case Manager Certification (CCMC) preferred.
• Strong knowledge of medical and behavioral health processes and ICD/CPT coding.

Responsibilities

• Collect and assess member information to promote wellness and cost-effective care.
• Coordinate resources to achieve member outcome goals.
• Document case notes and letters for legal records.
