Qualifications

• Ability to manage treatment planning and clinical documentation.
• Experience working with adult and geriatric populations.

Responsibilities

• Evaluate residents’ functional abilities and limitations.
• Develop individualized occupational therapy treatment plans.
• Help residents improve activities of daily living.
• Address strength, coordination, balance and functional mobility.
• Recommend adaptive equipment and environmental modifications.
• Educate residents, families and caregivers.
• Complete timely evaluations, progress notes and discharge documentation.
• Collaborate with nursing, rehabilitation and interdisciplinary teams.
• Participate in care planning and discharge preparation.
