Job summary

Fairfield Community Health Center in Lancaster, Ohio seeks a licensed Practical Nurse to join as a triage nurse. You will provide nursing care, triage by phone and in person, and manage medications and authorizations under supervision.

Proficiency with NextGen EHR is preferred, with a strong emphasis on patient safety and confidentiality. The role offers an opportunity to work in a team-based, patient-centered medical home with extended hours and a collaborative care approach to improve access

Qualifications

• Graduate of an accredited school of practical nursing.
• LPN license in the State of Ohio.
• BLS certification required.
• 2 years’ ambulatory health care or public health nursing preferred.
• Experience with medical records and EHR (NextGen) preferred.

Responsibilities

• Provides general nursing care to health center patients.
• Completes prior authorizations and/or pre-certifications.
• Documents patient encounters accurately in the EHR.
• Provides telephone and face-to-face triage for patients.
• Administers medications and injections as directed.
• Maintains patient confidentiality and safety policies.
• Assists front desk with scheduling and workflow.
• Manages communications with external offices and vendors.
• Demonstrates professional judgment and teamwork.
