Job summary

Rochester Regional Health is seeking a Triage Home Care Registered Nurse to facilitate access to the health care system and negotiate appropriate care options. The role requires collaboration with providers and the care team, using the nursing process and protocols to assess, plan, and communicate care plans to clients and caregivers.

The position necessitates NYS RN licensure, BSN preferred, and strong analytical, computer, and communication skills.

Qualifications

• Graduate from an accredited school of nursing.
• Current New York State Registered Nurse License.
• BSN preferred.
• Strong analytical, computer, and communication skills.
• Experience in emergency department, ambulatory care or telephone consultation is helpful.
• Medical terminology a must.
• Prior Home Care Experience a must.

Responsibilities

• Maintains the organization's standards of nursing care per policies and procedures.
• Using the nursing process, collects data, assesses condition, and formulates a plan of care.
• Negotiates appropriate nursing care options with client and caregiver and validates the plan.
• Collaborates with team members to implement the plan of care.
• Demonstrates excellent communication and customer-driven professional attitude.
• Provides non-judgmental advice based on protocols and resources.
• Demonstrates leadership in decision making, communication, and problem solving.
• Acts as client and family advocate to assist through the health care system.
• Attends staff meetings, in-services, and supervision conferences; performs related duties.
