Job summary

SSM Health seeks a Regular RN for visiting nurse services across the St. Louis area, coordinating care in patients' homes, CBRFs, assisted living locations, and facilities under physician direction. Travel is required to assess needs and implement care plans.

Responsibilities include discharge planning, comprehensive nursing assessments, documentation, and collaboration with the healthcare team to ensure quality end-of-life or discharge planning and adherence to regulations.

Qualifications

• Graduate of accredited school of nursing or education equivalency for licensing.

Responsibilities

• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
• May perform point of care testing according to policies and procedures.
• Collaborates with physicians regarding the plan of care and changes in patient status, documenting updates.
