Job summary

Spiras Health is seeking a Licensed Practical Nurse for a Multi-State Float role. You will travel to patients’ homes across multiple states, working with Nurse Practitioners to manage a defined patient population.

The position requires a valid multistate LPN license (NLC), 2+ years of LPN experience, and willingness to travel up to 50% with possible overnight stays. Remote-field mix includes home visits, assessments, and care coordination.

Qualifications

• Must have completed LPN licensing program.
• Minimum 2+ years of LPN experience.
• Active multistate LPN license (NLC) with primary state residency.
• Willingness to travel up to 50% and overnight stays.
• Valid driver's license and reliable vehicle.
• BLS for Healthcare Provider CPR certification.
• Experience with chronic conditions and home health care.
• Ability to use EMR and mobile health apps.

Responsibilities

• Travel to new territories across multiple states for startup and coverage, with home-office work half the time.
• Provide in-home and telephonic patient coverage in multiple states.
• Adhere to state-specific scope-of-practice and documentation rules.
• Complete state and market orientations before commencing care.
• Support onboarding of local clinical staff and ensure care plan adherence.
• Maintain nursing licenses and certifications; report changes.
• Conduct patient assessments, educate patients, and coordinate care with NPs.
