Job summary

JOL Healthcare seeks a Registered Nurse to provide in-home nursing care across diverse patient ages, coordinating with the IDT and referring agencies. You will perform comprehensive assessments, implement care plans, educate patients and families, and document progress.

Role requires current Texas RN license, CPR, and reliable transportation with ability to travel locally. Flexible schedule and travel to patients’ homes are expected.

Qualifications

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient and family.
• Coordinate care with the interdisciplinary team and referring agencies.
• Provide education to patient and family on disease process, self-care, end-of-life care, nutrition, and dietary needs.
• Prepare clinical notes and demonstrate progress toward goals.

Responsibilities

• Assess caregiver ability to meet patient needs at admission and during care.
• Coordinate services and prioritize needs with IDT members.
• Inform physicians and staff of changes in patient needs and responses to care.
• Travel locally to patients' homes and work on a flexible schedule.
