Responsibilities

• Assess patient needs and monitor signs of physiologic and psychosocial changes.
• Collect, analyze, and document nursing data and diagnoses.
• Document the plan of care with diagnoses, outcomes, and interventions.
• Initiate and evaluate patient and family education.
• Revise the plan of care based on evaluation and changes in medical plan.
