Responsibilities

• Assess patient needs and signs indicating physiologic and psychosocial changes; use nursing process to plan, implement and evaluate care.
• Collect, analyze, and interpret data; document nursing diagnoses.
• Document the care plan with nursing diagnoses, outcomes and interventions.
• Initiate and evaluate patient and family education.
• Revise and document the plan of care based on evaluation and changes in medical plan.
