Job summary

Northshore Home Health Care is seeking a Travel Speech-Language Pathologist (SLP) to evaluate and treat communication and swallowing disorders in patients in a home health setting. You will deliver therapy, document in a timely manner, and collaborate with healthcare teams to optimize outcomes.

The role requires a Master’s in Speech-Language Pathology, CFY and CCC, an active state license, and at least 1 year of home health experience, with a valid driver’s license and Homecare Homebase EMR

Qualifications

• Master's in Speech-Language Pathology from an accredited program.
• CFY completed with CCC from ASHA and current certification.
• Active state license to practice SLP.
• BLS certification may be required.

Responsibilities

• Deliver therapy services to patients and address communication and swallowing needs.
• Document thoroughly and adhere to clinical protocols while traveling to patient homes.

Job description

Role:

Travel Home Health Speech Language Pathologist (SLP) responsible for evaluating and treating communication and swallowing disorders in patients within a home health setting.

Key Responsibilities:

Provide therapy services to patients, improve communication skills, and address swallowing issues. Utilize the Homecare Homebase EMR system and adhere to clinical standards. Maintain documentation and collaborate with healthcare teams to optimize patient outcomes.

Qualifications & Skills

• Master's Degree in Speech-Language Pathology from an accredited program
• Completed CFY and current Certificate of Clinical Competence from ASHA
• Active State License required
• BLS Certification may be required
• At least 1 year of Home Health experience
• Valid Driver's License and proof of Insurance
• Proficiency with Homecare Homebase EMR

Key Duties:

Deliver therapy services during 8-hour shifts, typically from 8:00 AM to 5:00 PM, with no call or major holiday work. Focus on patient care, documentation, and adherence to clinical protocols in a travel capacity.
