Job summary

Centra Healthcare Allied is seeking a travel Speech Language Pathologist for a contract assignment in Millbrook, New York. The position is 13 weeks long, 35 hours per week, with dayshift hours in a SNF/LTC setting.

You must be an SLP-CCC, have graduated from an accredited program, and hold or be pursuing state licensure. You will collaborate with an interdisciplinary team to provide skilled therapy and meet physician orders.

Qualifications

• Current state license as a Speech Language Pathologist or in the process of licensure.
• Collaborate within an interdisciplinary team to develop a plan of care.
• Provide skilled speech therapy services per physician orders.
• Assess needs, plan, evaluate, and modify care for therapy goals.
• Set goals for communication and swallowing to support education/work/social roles.
• Candidate must be an SLP-CCC.

• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social roles
• Candidate must be an SLP-CCC.
• Experience with special needs preferred.
