Job Details

• Location: Seaside, Oregon area
• Shift: Days, 8 hours per day
• Schedule: 4 days per week, minimum 32 hours weekly
• Start Date: 10/19/2026
• End Date: 01/18/2027
• Certifications: BCLS (Basic Cardiac Life Support) required
• Years of Experience: At least 1 year of skilled home health experience preferred; 0 years minimum
• Patient Ratio Experience: Up to 6 patients per day in standard productivity, with a 1:1 ratio
• Qualifications: Valid state license, compact license preferred; must have own transportation; COVID-19 vaccination (primary series + booster) required; Oasis certification required; BCLS certification required
• Experience with charting systems: EPIC, Rover (phone app), Oasis
• Clinical skills include assessment, speech evaluation, communication devices, dysphagia management, and patient/family education
• Travel requirements include occasional driving up to approximately 60 miles daily.
• The provider must be flexible with scheduling, attend orientation, and meet the clients documentation and clinical proficiency standards.
