Qualifications

Responsibilities

• Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments for residents within the care setting.
• Develop and implement individualized treatment plans for speech, language, voice, cognitive-communication deficits, and dysphagia; monitor progress and adjust as needed.
• Provide direct therapy and supervise care plans in collaboration with an interdisciplinary team to support functional communication and safe eating and swallowing.
• Perform documentation including evaluations, progress notes, treatment plans, and discharge summaries in accordance with regulatory and facility standards.
• Participate in care planning and discharge planning, providing education to residents, families, and staff to support ongoing communication strategies and safe swallow practices.

Responsibilities

Requirements
