Healthforce seeks a Speech Language Pathologist for a 13-week travel assignment in Alameda, CA. Start date 11/16/2026. Workdays 8:00 AM to 4:30 PM (5x8) in a skilled nursing facility with acute care responsibilities.

Responsibilities include comprehensive assessments, therapy plan development, progress documentation, and collaborating with an interdisciplinary team to coordinate care and discharge planning.

Qualifications

• Ability to work in a skilled nursing facility setting.
• Commitment to a 13-week travel assignment.
• Strong written and verbal communication with accurate charting.

Responsibilities

• Perform comprehensive speech, language, voice, and swallowing assessments for residents.
• Develop, implement and monitor individualized therapy plans to address communication and swallowing needs.
• Deliver evidence-based treatment and document progress and outcomes.
• Collaborate with an interdisciplinary team to coordinate care and discharge planning.
• Participate in care conferences and contribute to progress notes and admin documentation.
