Job summary

Pioneer Healthcare Services is seeking a travel Physical Therapist for a Skilled Nursing Facility (SNF) assignment in Modesto, California. This 10-week travel role offers 40 hours per week in a day shift, with start date of 09/22/2026.

The PT will assess patients, develop treatment plans, and collaborate with the care team to support mobility and independence. CA license and BLS/CPR are required; on-site work with weekly pay and comprehensive benefits.

Qualifications

Doctorate in Physical Therapy.
CA Physical Therapist license.
Current BLS/CPR Certification.

Responsibilities

Assess patients' physical abilities and develop personalized treatment plans.
Provide therapy to improve mobility, strength, and functional independence.
Collaborate with healthcare teams to ensure comprehensive patient care.
Educate patients and families on exercise programs and injury prevention.
Monitor and document patient progress and adjust treatment plans as needed.
Implement interventions in various settings.
Participate in discharge planning and coordinate follow-up care for continued rehabilitation.
