Qualifications

• Current RN license for the state of practice.
• BLS certification required or current.
• Health certificate current per facility requirements.
• One year RN experience preferred.

Responsibilities

• Uses the nursing process to plan, implement, and evaluate care for clients on contracted visits.
• Initiates plan of care and requests revisions as patient needs change.
• Administers medications and treatments as prescribed and documents effects.
• Monitors patient status and reports changes to physician or supervisor.
• Performs care coordination, education, and discharge planning as applicable.
