Qualifications

• Minimum of 2+ years of clinical experience in a related setting.

Responsibilities

• Provide patient care support during 12-hour shifts, totaling 36 hours weekly.
• Ensure safe care transitions through effective handoffs and care plan updates.
• Collaborate with physicians and healthcare team members to coordinate patient activities.
• Educate patients and families regarding care plans and discharge procedures.
