Responsibilities

• Assesses patient needs, signs and symptoms indicating physiologic and psychosocial changes in the patient's condition using the nursing process.
• Collects, analyzes, and interprets data and documents nursing diagnoses and care plans.
• Documents the plan of care with outcomes and interventions.
• Initiates and evaluates patient and family education.
• Revises the plan of care in response to changes in clinical status.
