Qualifications

Responsibilities
- Plan of care interventions including medication administration and treatments.
- Monitor and record patient condition and communicate with care team and family.
- Coordinate patient care across disciplines; arrange transfer, discharge, referrals, and psychosocial support.
- Educate patient/family according to age, culture, condition, and circumstances.
- Advocate for patient physical and emotional well-being.
