Job summary

AMN Healthcare Revenue Cycle is seeking a travel nurse RN Case Manager in Fort Worth, TX for a 13-week assignment. Work 40 hours per week with a consistent 8:00 AM–4:30 PM shift as a Travel RN Case Manager.

You will coordinate patient care from admission through discharge, collaborate with physicians, nurses and external providers to remove barriers to care, and support safe, efficient discharge planning using Meditech and Careport.

Qualifications

• BLS credential required.
• CCM certification preferred.
• TX/compact RN license required.
• 1+ year case management experience in an acute hospital setting.

Responsibilities

• Coordinate patient care from admission through discharge, ensuring treatments and resources align with each patient’s medical needs and recovery goals.
• Collaborate with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.
