Qualifications

• Eighteen months of recent experience in an Acute Care Case Manager setting.
• Active RN License.
• BLS Certification.
• Degree from accredited nursing program.

Responsibilities

• Coordinate continuity of care for patients between family and healthcare providers.
• Work across diverse settings including HMOs, community health, long-term care, behavioral health, rehabilitation, schools.
• Promote self-managed care and efficient use of healthcare resources.
• Ensure timely administration of appropriate treatments to maximize patient well-being.
