Job summary

Prodware Solutions LLC is seeking a travel nurse RN Case Management for a travel nursing job in Eureka, California. You will perform comprehensive patient assessments, coordinate care, and implement effective discharge plans to ensure smooth transitions across settings.

This role requires 2+ years of acute care Case Management, experience with Epic, and strong coordination across interdisciplinary teams. Travel experience preferred; 13-week assignment with dayshift.

Qualifications

• Minimum 2 years of acute care hospital Case Management experience.
• Recent acute care Case Management experience within the last 12 months is mandatory.
• Experience in care coordination and discharge planning.
• Previous experience working with Epic documentation system.
• Travel experience preferred; first-time travelers may be screened with approval.

Responsibilities

• Perform comprehensive patient assessments and identify discharge planning needs.
• Coordinate patient care and facilitate smooth transitions across healthcare settings.
• Develop and implement effective discharge plans.
• Conduct utilization review and evaluate admission criteria.
• Coordinate with physicians, nursing staff, social services, and interdisciplinary teams.
• Manage insurance authorizations, benefits eligibility, and care coordination.
• Handle appeals and denials and ensure appropriate utilization of healthcare resources.
• Assess patient needs and coordinate Durable Medical Equipment (DME) arrangements.
• Ensure compliance with CMS guidelines, HIPAA regulations, Joint Commission standards, and patient safety requirements.
• Maintain accurate documentation and case management records in Epic.
