Job summary

Qualifications

• Familiarity with HMOs, IPAs, and insurance providers.

Responsibilities

• Coordinate patient care and discharge planning in acute hospital settings.
• Perform comprehensive patient assessments and identify discharge barriers.
• Arrange post-acute services including SNF, home health, and DME.
• Conduct utilization reviews and document medical necessity.
• Coordinate with HMOs, IPAs, and insurers on coverage and authorizations.
• Assist with appeals, denials, and insurance-related discharge issues.
• Document activities in Epic and ensure CMS/HIPAA compliance.
