Job summary

TotalMed RN is seeking a Travel Nurse RN Case Manager for Acute Care Case Management in Oklahoma City, OK. This 13-week travel assignment requires 40 hours per week, with 10-hour day shifts.

The RN case manager will assess patients, plan care, educate families, and coordinate with the care team to improve outcomes and reduce readmissions. The ideal candidate has 1–2 years of case management experience, BLS certification, and the ability to manage fluctuating caseloads while communicating

Qualifications

• BLS certification required.
• 1 year of case management experience preferred.
• 13-week travel assignment.

Responsibilities

• Evaluate and assess new patients and needs.
• Create and update comprehensive short- and long-term care plans.
• Educate families and patients about resources and options.
• Collaborate with RNs, physicians and social workers.
• Document and submit medical records accurately.
• Communicate with insurance to coordinate or appeal decisions.
• Analyze outcomes to identify trends and inform management decisions.
