Aya Healthcare, Inc. has an immediate opening for a Case Manager Registered Nurse in New London, CT. The RN will manage patient care, perform biopsychosocial assessments, and coordinate services within an acute care setting using Epic for charting.

Requirements include CT licensure or compact license and at least 1 year of experience. The assignment is 13 weeks, days shift, with a start date of 11/02/2026 and a weekly pay range of $2,386 to $2,589.

• Experience in case management and social work within healthcare settings.
• Proficiency in completing intake and admission screenings and coordinating care.
• Knowledge of regulatory and documentation standards for nursing home residents.
