The RN Case Manager coordinates care for hospitalized patients with complex medical and social needs, focusing on assessment, discharge planning, care coordination, resource utilization, and safe transitions of care.

The role is a 13-week travel assignment, 40 hours per week, with an 8-hour shift. Start date is 10/05/2026 and pay rate ranges from $1600.00 to $2500.00 per week.

Qualifications

• Active Montana RN or Compact License.
• Three (3) years of experience as a Registered Nurse in acute care.
• Knowledge of Medical Terminology.
• Proficiency with Microsoft PowerPoint and Excel.

Responsibilities

POSITION SUMMARY

MINIMUM REQUIRED QUALIFICATIONS

• Active Montana RN or Compact License
• BSN
• Three (3) years of experience as a Registered Nurse is required, acute care setting required.

PREFERRED QUALIFICATIONS

• Strong discharge planning and care coordination experience
• ACM or CCM preferred, but not required

13 weeks SHIFT / HOURS PER WEEK 5 days/week 8:00a-4:30p (40hrs) START DATE 10/5/2026
