Qualifications

• MO/compact RN license required.
• BLS certification required.
• 2 years case management experience in an acute hospital setting.
• Acute hospital experience in specialty during the past year with no more than a 90‑day break.
• Recent acute discharge planning experience.

Responsibilities

• Coordinate patient care from admission through discharge, ensuring treatments, services, and resources align with each patient’s medical needs and recovery goals.
• Collaborate with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe discharge planning.
