The purpose of the Case Manager I position is to support the physician and interdisciplinary team in facilitating patient care. This role focuses on enhancing the quality of clinical outcomes and patient satisfaction. The Case Manager I is accountable for a designated patient caseload and plans effectively to meet patient needs. This position involves managing the length of stay and promoting efficient utilization of resources. Facilitation of precertification and payor authorization processes is a key responsibility. The role includes coordinating communication with physicians and intervening as necessary to remove barriers to timely and efficient care delivery. The Case Manager I will apply process improvement methodologies in evaluating outcomes of care. Support and coaching of clinical documentation efforts are also part of this role. The candidate should demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Knowledge of principles of growth and development as it relates to different life cycles is essential. Preferred skills include a current working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
