Job summary

Porter Cares is expanding its reach into New Jersey and seeks experienced Nurse Practitioners to join our statewide travel program. This role supports patients across New Jersey, with 50–100% of the state covered.

You will perform in-home health risk assessments, review medical histories, manage medications, and contribute to care coordination with Porter Care Guides. A NJ NP license and 3 years of patient care are required.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience required (primary care/adult/geriatric).
• Strong empathy, compassion and communication skills.
• Experience with EMR systems preferred.

Responsibilities

• Visit members within geographic area and perform in-home health risk assessments.
• Review past medical history, medications, vital signs, and conduct a comprehensive exam.
• Coordinate care with Porter Care Guides and active medical management.

What You\'ll Do

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.

• Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion
