Job summary

Healthforce is seeking a Registered Nurse for a long-term care/post-acute rehab unit in Middlebury, VT, starting 11/09/2026 for 16 weeks. Night shifts are 12 hours (3x12). Weekly pay ranges from $2,081 to $2,153, with potential extension.

You’ll provide direct nursing care, administer meds and IV therapy, document in EPIC, and collaborate with therapy and care coordination to support discharge planning. Travel support and orientations are provided.

Qualifications

• Active RN license in Vermont (or eligibility to practice in VT).
• BLS certification.
• EPIC experience within the past 2 years (nonnegotiable).
• Minimum 1 year of clinical experience; post-acute/SNF/skilled nursing rehab preferred.
• Prior travel experience required.
• Dementia-capable care certification preferred.
• Ability to work a 12-hour night shift; floating to other units as needed.

Responsibilities

• Provide direct nursing care to post-acute skilled nursing residents, including assessments and ongoing care planning.
• Administer medications and IV therapy as ordered; monitor IV pumps and patient response.
• Document comprehensive assessments, interventions, and progress in EPIC; collaborate with the interdisciplinary team.
• Perform phlebotomy, EKGs, and patient transports; assist with Nebulizer to inhaler transitions and unit procedures as assigned.
• Collaborate with unit leadership, therapy, and care coordination to support discharge planning and goal attainment.
• Maintain patient safety and comfort; implement falls prevention and dementia-capable care practices as appropriate; respond to emergencies.
