Job summary

Gentiva Hospice is seeking a Hospice RN - Regional Support to provide direct patient care and support to families, serving as a floating clinician between branches based on census needs. This role includes on-call responsibilities and requires travel to multiple Gentiva locations.

The position offers a Monday–Friday schedule with travel within the region, and a comprehensive benefits package along with base pay in the advertised range.

Qualifications

• Current, unencumbered RN license in the state of employment.
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting).
• Current CPR certification.

Responsibilities

• Deliver high-quality nursing care for hospice patients.
• Manage and implement individualized plans of care.
• Lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
• Assess patients’ physical, emotional, spiritual, and environmental needs.
• Document care and ensure compliance with hospice protocols.
• Educate families on care instructions, medications, and safety.
• Participate in on-call rotation and after-hours care as needed.
• Collaborate with physicians, referral sources, and the care team.
• Assist with referrals, regulatory documentation, and payer compliance.
• Travel to Gentiva hospice locations as assigned to fill urgent needs.

We’re looking for a Hospice RN - Regional Support to join our team. In this dynamic role, you’ll provide direct care to patients and support to families, while also serving as a floating clinician - temporarily assigned to different hospice branches based on census needs or staffing gaps. As a Hospice RN - Regional Support, You Will:

• Deliver high-quality, compassionate nursing care for hospice patients
• Manage and implement individualized plans of care
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)

Schedule & Travel:

This role typically follows a Monday-Friday schedule, with occasional on-call responsibilities. Travel is required and may include:

• Option 1: Overnight travel required to cities within a 100-mile radius.
• Option 2: Frequent overnight travel to multiple states is required.
• Option 3: Regional travel with overnight stays based on assignment needs.

A company fleet car is provided for eligible employees.

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range - $89,000 to $94,000

About You

Qualifications – What You’ll Bring:

• Current, unencumbered RN license in the state of employment
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
• Current CPR certification
• Valid driver’s license, auto insurance, and reliable transportation

Preferred Experience (Not required):

• Hospice and end-of-life nursing experience
• Skilled in clinical assessments and compassionate communication
• Strong critical thinking, flexibility, and problem-solving skills
• Comfortable working in varied environments with shifting needs
• Proficient in documentation and electronic health records
• Calm and supportive in crisis situations; skilled at family engagement
